Accident / Medical Waiver & Release

By booking a session with KCP Softball, you acknowledge and agree to the following:

• You grant permission for your athlete(s) to participate in KCP Softball’s private coaching sessions.

 

• You confirm that, to the best of your knowledge, your athlete is physically and mentally able to participate in these activities.

​​

• You authorize KCP Softball to use your athlete’s likeness, name, voice, or words in photographs, videos, and promotional materials, including for advertising or grant applications.

​​

• You understand that KCP Softball is not liable for any injury or disability that may occur as a result of participation, and that you assume all associated risks on behalf of yourself and your athlete.

 

In the case of a medical emergency where the parent/guardian is not present or cannot be reached, KCP Softball is authorized to take appropriate measures to ensure the athlete receives emergency medical care, including hospitalization if necessary.

By participating, you agree that you are the parent or legal guardian of the athlete and that you have read, understood, and accepted all terms of this waiver.

Cancellation Policy

To respect our coaches’ time and ensure fairness for all clients, KCP Softball enforces the following cancellation policy:

​

• A 50% cancellation fee will apply to no-shows or late cancellations.

​

• You may be considered a no-show if you arrive more than 15 minutes late to your scheduled appointment.

​

• To avoid a cancellation fee, cancellations must be made at least 24 hours in advance.

​​

• Cancellation fees are only charged when terms of this policy are violated.
